  • Based on the membership level, one, three, or unlimited same‑day or same‑week primary care visits (in-person & telehealth)
  • Direct clinician messaging and priority responses
  • Annual comprehensive exam with personalized prevention plan
  • Rapid lab ordering and at‑home test coordination (HSAT included)
  • Medication management and close follow‑up (including GLP‑1 titration support)
  • Care coordination and expedited specialist referrals (note: specialist costs billed to insurance)

    • A: Membership covers the clinician’s time and coordination. Labs, imaging, and prescription costs are billed to insurance or pharmacy as usual; we assist with prior authorizations and paperwork
  • Q3: Can I join if I already have Empire BlueCross BlueShield?
    • A: Yes — many of our members maintain insurance and add DPC to gain faster access and longer visits
  • Q4: How do I switch between insurance visits and membership care?
    • A: We’ll explain billing and the best path during your membership consult; transitions are straightforward, and we’ll support coordination
